Skilled Nursing Facility Limited Visitation Protocols (08/10/2020)

Dear Family, Friends and Residents,

We are happy to notify you that our facility has met all the CMS, CDC, and DOH guidelines to begin
limited visitation. Among other requirements, our facility has developed a NY Forward Safety Plan
and have been COVID-19 free for 28 days.

At this time outdoor visitation only will begin Tuesday, August 11, 2020. Visitation will be held
Tuesday-Thursday 1pm-4pm, weather permitting. If you wish to visit your loved one you must call
ahead (preferably 24 hours in advance). Appointments will be made through our Social Service
Department only. Please do not call the receptionist or unit as they will not be able to schedule a
visitation appointment for you, but will take every effort to connect you with the appropriate
department. You may contact either Maggie Lombardo at (845) 878-3241 ext. 270 or Nicole Burden at
ext. 214 to schedule your visit. Please keep in mind visitation may have to be re-scheduled due to
poor weather conditions.
